New participation forms are required for each out-of-county event. One copy must travel with the participant. Participation forms serve a number of purposes. Before participating in an out-of-county event parents need to send the 4-H Office updated contact information. Health conditions can appear and change from event to event. By signing a new form, the participants agree again to the conditions of the Code of Conduct. Parents can agree or disagree to authorizations listed on the back page for each event.
